To receive your Payback Bonus by mail, follow these instructions:

1. Purchase qualifying products from two or more brands between 1st October 2005 and 31st December 2005.
2. Remove the UPC or EAN including serial numbers (if applicable) from packing cartons for each of your qualifying product purchases. Only original UPC or EAN and serial numbers will be accepted.
3. Calculate the value of your expected rebate by carefully completing the offer form on the Payback claim form (available on request).
